At Foundation Sport & Spine, we accept most forms of major insurance.  We are currently in-network with PPO plans, such as Blue Cross Blue Shield, Blue Choice Plans, and Aetna plans.

 

We submit to Medicare as well, however, Medicare patients will receive reimbursement directly from Medicare, and we will collect payment up front (but treat you as a self payer with the appropriate discounts, as described below).  Medicare pays for the chiropractic manipulation, and your secondary may pick up other non-Medicare-covered charges (i.e. exams, xrays, and therapeutic exercises).

 

We also treat MANY patients who have United Healthcare, Cigna, Humana, and other insurance companies, and we work with these patients and their insurance companies as such:

-Patients are offered self-pay discounts up to 25%, depending on payment method (i.e. payment plan or payment up front)

-Patients who request detailed invoice from our office are able to submit it to their insurance company for out -of-network benefits.

MANY patients have received significant out-of-network payment for their visits, directly from their insurance company this way.

-Some patients who receive out-of-network benefits, along with our self-pay discounts pay LESS for their treatment plans than those who are in-network with Blue Cross.  So please keep this in mind as you consider care with us!
